This page provides information for persons who are completing on-line appraisal forms:
LOGGING IN
- When I go to the appraisals page I'm asked for a Username and Password but I don't have one or have forgotten it.
- Nearly all institutes develop their own unique appraisal formats in conjunction with Jolin Consulting. To protect privacy and copyright, we use passwords to access all individual institute sections of this site. To obtain a username and password, you must contact your principal.
- I'm having trouble entering my Username and Password to log into my institute.
- Check you have clicked on your institute and do not have Caps Lock on - all usernames and passwords are lowercase.
FILLING IN A FORM
- What preparation should I do?
- You should already be familiar with the criteria relevant to the appraisal you are completing. Ideally you will have some notes at hand to help you address each criteria specifically for the person being appraised.
- How long will it take?
- A typical on-line appraisal form usually takes around 20 to 30 minutes to complete, although more involved appraisals may take up to an hour.
- Can I stop halfway through?
- Yes, but we recommend completing a form in one session. If you run out of time before completing any form, first print a copy and then click the SUBMIT button. When you return at a later date, carry on filling out the form from where you left off (using your printed copy as a reminder) and then SUBMIT again. (There is no need to re-enter data previously submitted.)
- Must all parts of the form be completed?
- Yes. You will always need to fill in the appraisee details at the top of the form before submission is possible. Warnings may appear if you omit a rating or comment for any of the criteria.
- How will I know if the form is sent OK?
- After you click on the submit button, carefully read and respond to any on screen messages about errors in filling out the form. If all is OK, a new page will appear indicating that the form has been sent successfully and giving you a receipt number to record.
- The appraisal says the criteria are in PDF format, but I don't have Adobe Acrobat to read this type of file
